3. Has Spawn: Origins Collection Hardcover Volume 9 been announced for release yet?
4. Is the Medieval Spawn in the early Spawn issues the same guy as the one in the Medival Spawn & Witchblade team up?

1. Short answer, yes. There are quite a few small appearances you have not listed. However, the vast majority of them (if not all) do not have any bearing on core continuity. I have been working on a large update for the website and will be including many cameo/guest appearances soon!
2. Continuity is (at this point) a subjective thing. There is no "official" list. However, based on the comments of Spawn's writers over the past several years, most of the spin-offs and crossovers should not be considered core continuity.
For the purposes of the Spawn encyclopedia that I keep, I use the following internal rules:
- The main series "Spawn' is always in-continuity unless a later issue has contradicted an earlier issue, in which case the later issue takes precedent.
- Mini-Series and spin-offs published by McFarlane are always in-continuity, unless they have been contradicted by the main series which takes priority.
- Mini-series and spin-offs published in Image comics but not published by McFarlane are not in continuity unless it is an anthology book (ie. Image Comics Hardcover) or Image United.
- Spawn appearances in other Image characters' books (ie. Savage Dragon, Younbgblood) are not in-continuity.
- Spawn appearances in books by other publishers are not in-continuity.
Those are the rules I use to help make sense of things, yours may vary, but they work for me
